• Place and Date of Birth 
• Parent’s full names including mother’s maiden name
• Total years of formal education 
• Place of employment and occupation
• Spouse’s date of birth and Social Security number (if surviving) 
• Cemetery information

• Clothing of choice
• Undergarments 
• Jewelry (watch, ring, earrings, etc.) 
• Eyeglasses 
• Socks/Hosiery 
• Shoes & Belt (optional) 

• Life Insurance Policies
• Funeral Home pre-need policies

• Honorable Discharge (DD214)

• Photograph for the obituary
• Please bring approximately 40 photos spanning your loved one’s life, including photos from childhood, vacations, military service, weddings, and family. We encourage you to choose photos of the way you wish to remember and honor your loved one. 

• Where will the service take place? (Funeral Home Chapel, Church, Graveside...)
• Who will officiate the service? (pastor, chaplain, friend or family member) 
• Pallbearers (6 persons) 
• Special Music - Are there any favorite songs to be played or sung during the funeral services? 
• Memorial Contributions - Favorite charity(s) to which people can make memorial donations. 
• Personal items to be displayed during the visitation or service. 
• Should cremation be your choice, will you be honoring your loved one with a memorial service?
